This study aims to assess quantitative uses of SDOH among patients at-risk for or diagnosed with type 2 diabetes and assess providers who see patients at-risk for or with type 2 diabetes, and other key stakeholders to understand factors that influence the use of SDOH and at BJC to inform future dissemination and implementation strategies.
